We make every effort to provide you with the finest care and the most convenient financial options.  We accept a wide variety of dental PPO plans, and our office will handle billing your insurance company.  Your co-payment is due at the time service is rendered unless other arrangements have been made in advance.

Scheduling


Our office is open Monday through Thursday from 8:00am to 5:30pm.   


Your appointments are specially blocked out for you; thus please give us 48 hours notice should you need to reschedule or cancel.  Should a dental emergency arise, every attempt will be made to see you that day.

Patient Information



First Visit


Your initial appointment will consist of a consultation explaining your diagnosis (done via evaluation of digital dental radiographs/intra-oral camera and a comprehensive oral exam), followed by our proposed treatment plan to remedy all concerns. If time permits, treatment may even be performed the same day as the consultation. However, a complex medical history or treatment plan will more than likely require multiple appointments.


Please assist us by providing the following information at the time of your initial visit:


  • Radiographs (X-Rays). If you have radiographs from a previous dentist or physician, you may request that they forward them to our office.  This process could take a couple of weeks, so if there is not enough time to mail your radiographs before your scheduled appointment with us, you can arrange to pick them up in person.  Should additional films be necessary, they can be taken at our facility.
  • A list of your current medications and list of any known allergies
  • Dental/medical insurance information
  • Referral slip from your physician or specialists (if applicable)


IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (e.g. heart medications, aspirin, anticoagulant therapy, etc.) .
